A large part of the United States lacks access to one or more kinds of health and physical rehabilitation services creating “medical deserts.” Underserved communities residing in these medical deserts face various social and economic barriers to receiving timely, high-quality, comprehensive post-acute rehabilitation services. In this FFX Talk, Dr. Amit Kumar will share reflections and lessons learned from his efforts to develop community partnerships with community health providers and CHRs to build a deeper understanding of the unmet needs of Native Americans accessing rehabilitation services after hospital discharge and improve their health outcomes
